Mental Health Day Programs

FREE provides specialized, strength-based, recovery-oriented day services to individuals ages 18 and over with severe and persistent mental illness. Services designed to promote a happier and healthier life.

Personalized Recovery Oriented Services

Personalized Recovery Oriented Services (PROS) participants (18-65) attend during the day to work on life goals, such as returning to work, minimizing relapse, or improving their social connections to the community. PROS offers a full array of services including community rehabilitation and support, intensive rehabilitation, ongoing rehabilitation, and support, and clinical treatment. Individuals attend 1-5 days each week depending on their Individualized Recovery Plan.

Individuals work towards their own unique goals, such as:

  • Living Independently
  • Building Natural Supports
  • Maintaining Employment
  • Obtaining Higher Education
  • Securing preferred Housing
  • Improving Medication Management

 

Community Oriented Recovery and Empowerment Services

FREE offers Community Oriented Recovery and Empowerment services (CORE), person-centered recovery-oriented mobile behavioral health supports. These services are available to adult Medicaid beneficiaries with serious mental illness and/or addiction disorders to receive these services in their own home and/or at our facilities.

FREE Provides These CORE Services:

Community Psychiatric Support and Treatment (CPST) – These services assist individuals in developing essential life skills such as building social connections, securing and maintaining employment, initiating or resuming education, and effectively utilizing community resources.

Psychosocial Rehabilitation (PSR) – PSR services aim to help individuals enhance their functional abilities to the fullest extent within their living, working, learning, and social environments. Through rehabilitation counseling, skill development, and interventions, PSR is utilized to foster the achievement of person-centered recovery goals and meaningful life roles. These services can also address educational and employment related needs.

Family Support and Treatment Services (FST) – FREE offers instruction, emotional support and the skills necessary to build engagement and active participation of the family.

Empowerment Services – Peer Supports – Non-clinical, peer-delivered services with focus on rehabilitation, recovery, and resilience. These services promote skills for coping and managing behavioral health with facilitating the use of natural supports and community resources.

Home and Community Based Services

FREE offers Adult Behavioral Health Home and Community Based Services (HCBS). These services are available to people aged 21 and over and who are enrolled in a Medicaid Managed Care (MMC) Health and RecoveryPlan (HARP), and are found eligible after completing the NYS EligibilityAssessment. People enrolled in a Special Needs Plan (SNP) may also be eligible for HCBS.

Home and Community Based Services Include:

Prevocational Services – These services are aimed at preparing an individual for employment. Our training program prepares people for a career by working on clerical skills, job skills, interviewing, resume writing, team building skills, community relationships, and how to dress for success.

Intensive Supported Employment – This program is designed to help individuals secure and retain competitive employment. Participants will receive support aimed at helping them learn new job skills or sustain their current position.

Ongoing Supported Employment – This service is offered once a participant successfully secures competitive and integrated employment and completes orientation. Ongoing follow-along support is available for an indefinite period, as needed by the participant, to help them sustain their paid employment position.

Education Support Services – Assistance with tutoring, obtaining a GED, certifications for employment, and assistance with college applications including applications, financial aid and scholarships.

Habitation – These services are available to help foster connections with the community and to create connections to other available services. Daily life skills are also taught including finances, dieting, and living space maintenance.
